If you have OfficeMate open in addition to an exam in ExmWRITER and also have the ExamDRAW program open, you may receive error 360 when you close OfficeMate using the "X" in the upper right corner of the window. This error will appear after all of the programs (OfficeMate, ExamWRITER, and ExamDRAW) close. Eyefinity is aware of this error in version 7.4 and is working to eliminate it in a future version.
Error 360
Errors 400, 189183, & 418105 (Main Resolution)
If you are receiving error message 400, "error handler error 28 out of stack space procedure name: sub_center window," or errors 189183 or 418105, there was an invalid request and the computer is completely unable to parse it. You may be experiencing network problems, such as a time-out, or the request was indecipherable.
If you are receiving an error 400, 189183, or 418105 message while printing, especially while printing CMS 1500 forms or Rx Order forms, ask your IT professional to perform the following actions to resolve the issue:
- Open the virus software on each computer experiencing the error and disable the 'script blocking,' 'watch for suspicious activity,' or 'enable behavior monitoring' setting (the location of this setting can vary and is dependent on your anti-virus application).
- Use a business-grade PCL5e-compatible laser printer (preferably an HP LaserJet).
- Download and install the most recent PCL5e printer driver for the specific printer from the manufacturer’s Web site.
- Add the following lines to the [System] section of the Omate32.ini file:
- LABORDERPRINTER=<name of printer>
- MEDRXPRINTER=<nameofprinter>

Correcting Error 429
If you receive an error 429 message, ensure that you are meeting the following conditions:
- You are logged into the computer with the correct permissions and administrative rights. If you need help setting up permissions and administrative rights, contact your network administrator.
- You have installed the most recent Windows service packs on your computer and server. To install the most recent service packs, go to http://windowsupdate.microsoft.com.
- If you are using a Dymo LabelWriter, you have the printer's software (not just the driver) installed on the computer and network (if applicable).
If you receive an error 429 message while installing OfficeMate Server to upgrade a new instance of Microsoft SQL Server 2008 R2 Standard, ensure that the Microsoft SQL Server 2005 Backward Compatibility Components are installed on the server. Click here for more information on these components.
If you receive an error 429 message while you are using the Patient Ledger, go to the "Correcting Error 339" Knowledge Base article (linked below) and complete the instructions in the article.
If you are using OfficeMate/ExamWRITER v8 or below and you receive an error 429 message that states that the ActiveX component cannot create the object, ensure that you are logged into the computer as an Administrator, navigate to your OMATE32 folder, and double-click on the ~OMRegAll.bat file to re-register all of your OfficeMate/ExamWRITER files.

Unable to Select a Status When Recording an Order for the Lab Interface
When recording an eyewear lab order, you may receive the following message about invalid data:
You cannot select the status Transmitted to Lab (or the name of the status you selected on the lab order). Only applies to Lab Interface.
This warning displays if you selected a status from the Status drop-down menu on the Eyewear Order window that is tied to the Lab Interface, or if you selected a status that only the Lab Interface can assign.
If you receive the invalid data message, review your lab order transmit and result statuses and then select the appropriate lab statuses by following the instructions below:
- From the OfficeMate Administration main window, click Setup and select Preferences.
- Click the Centralized tab.
- Review the Order Transmit code. This is the status you must select when recording an order so that the order is sent through the Lab Interface.
- Review the Order Result code. This is the status the Lab Interface assigns to an order after it has been sent to the lab. Do not select this status when recording an order.
OfficeMate Has Detected That You Are Running on a Terminal Services Environment (ERA)
Error 380 (Lens Product Loader)
If you receive error 380 (Form Name: frmChangedProducts; Procedure: Fun_UpdateDiscontinuedProducts) while attempting to discontinue ophthalmic lenses in the Lens Product Loader, deactivate the lenses instead from the Products window (Product Details tab).
Error 70: Permission Denied
Error 70: Permission Denied with ECR Vault
You may receive Error 70: Permission Denied when, in a patient's record in ExamWRITER, you open the Images, Drawings, and Graphics category and attempt to open an image from the ECR Vault.

Error 70: Permission Denied with the VSP Interface
You may also receive Error 70: Permission Denied when you are attempting to obtain a VSP authorization from within OfficeMate. To resolve this error, complete the tasks below:
- Ensure that your permission are set up properly using the attached related article below
- Temporarily disable any anti-virus or other file protection software.
- Locate and move the VSPSentlog file to a temporary folder; OfficeMate will recreate this file when attempting to obtain another authorization.
Error 70: Permission Denied when Copying Products
You may also receive Error 70: Permission Denied when you are copying a product in OfficeMate/ExamWRITER that has a patient fee and you are logged on as a Terminal Server user. To resolve this error, complete the tasks below:
- Allow users to change the system time (Open the Group Policy Editor>Computer Configuration>Windows Setting>Local Policies>User Rights Assignment>Add users to Policy to "Change the system time.")
OR - Remove the patient fees from the product.

If you receive error -214623060, Invalid Object Name 'Audit_EventType,' in OfficeMate v10, it may be because you are using a backup program that does not specialize in backing up SQL databases or you may have more than one backup program running at the same time. See the attached article about property backing up databases.
To resolve this error, navigate to your OfficeMate or OMATE32 folder on the server (or workstation) where OfficeMate is installed and double-click AuditDBConversion.exe to resolve the error. If you cannot find this file, or if double-clicking it does not resolve the error, contact Customer Care at 800.942.5353.
Note: You only need to run this file on the server (or one workstation) where OfficeMate is installed one time.
This fee slip has been closed and cannot be edited.
If you are using OfficeMate v10.5 or above and you receive a message after creating a new fee slip that states, "This fee slip has been closed and cannot be edited," follow the instructions below for each location to resolve the issue and ensure that you can edit the fee slip:
- Open OfficeMate Administration.
- Click Setup and select Location Maintenance.
- Double-click on a location.
- Deselect the Charge Interface On check box.
- Click Save/Exit.

SQL Database Consistency Errors May Indicate Corruption
If your SQL database has consistency errors (possibly discovered by DataHEALTH during a backup or while running DBCC check) that you believe may indicate corruption, follow the troubleshooting tips below:
- Read Microsoft's knowledge base article at http://support.microsoft.com/kb/2015748 to familiarize yourself with the issue.
- Restore your data from a backup that has no errors from CHECKDB after all hardware and network issues have been addressed.
- If restoring your data from a backup is not possible, take Microsoft's course of action as described in the knowledge base article at http://support.microsoft.com/kb/2015748.
Error 2147467259 - "Cannot open Microsoft Jet Engine workgroup information file"
If you receive a "Cannot open Microsoft Jet Engine workgroup information file" message when opening OfficeMate or ExamWRITER, it may be due to a conflict with your virus protection software. To resolve this issue:
- Pause or stop your virus protection software.
- Delete or rename the system.mdb file.
- For OfficeMate, the file is located on your desktop.
- For ExamWRITER, the file is located inside either the OMATE32 directory or the OfficeMate directory.
Note: The system.mdb file has a file size of 0 bytes.
Error iErr=3-2147217900 (Meaningful Use)
Error -2147024894 (Logging into OfficeMate/ExamWRITER While Running Avast)
Problem: Avast recently released an update to their antivirus software and it is now causing error -2147024894 to appear for some users when they attempt to log into OfficeMate/ExamWRITER.
Resolution: If you are receiving this error and are unable to log into OfficeMate/ExamWRITER after receiving the Avast update, follow the instructions below:
- Restore the quarantined files from the Avast Virus Chest.
- Add the quarantined files to the list of exclusions.
- Add your entire OfficeMate directory (typically C:\OMATE32 or C:\OfficeMate) to the list of exclusions.
Note: Contact your IT professional to ensure that your Avast antivirus software is set up properly to exclude the OfficeMate directory from being quarantined after future Avast antivirus software updates.
Errors 400, 189183, & 418105 (Secondary Resolution)
If you receive error 400, error 189183, or error 418105 while attempting to print an Rx order or medical Rx order and you cannot resolve the issue using the methods in the knowledge base article at http://www.officemate.net/omkb/article.aspx?id=10763, ask your computer technician to follow the instructions below to output the Rx order to a PDF and then send the PDF to your printer:
Note: You do not need Adobe Reader to complete these instructions.
- Go to http://sourceforge.net/projects/pdfcreator/ and download and install PDFCreator.
Note: If you are asked if you want to install any extra programs, like the Yahoo! Search Toolbar or Qihoo 360, click Decline. - After the PDFCreator installation completes, open PDF Creator (Start>Programs>PDFCreator>PDFCreator).
- Click Printer and select Options.
- Click Auto-save on the left side of the window.
- Select the Use Auto-save check box.
- Type a static filename, for example OMTEMP, in the Filename text box.
Note: Using a static file name ensures that there will always be no more than one PDF file in the temporary output directory and that you will not need to manually delete files. - Select the Use this directory for auto-save check box.
- Navigate to and select a temporary output directory for Rx orders (for example, C:\Documents and Settings\username\Desktop\RxTemp).
- Click Print on the left
"EHR_equipif.main.err=p" or "EHREquip.Main.Error=9" (Eyefinity EHR Integration with Reichert RK600 Autorefractor/Keratometer)
Issue: After upgrading to OfficeMate 12.0, while using the Eyefinity EHR equipment interface to integrate with the Reichert RK600 Autorefractor/Keratometer, you may receive an error that states, "EHR_equipif.main.err=p desc subscript out of range" or "Subscript out of range. EHREquip.Main.Error=9."
Resolution: Uninstall the equipment interface (if it is installed twice, uninstall both instances) and then download and install the patch in the article at http://www.officemate.net/omkb/Article.aspx?id=34123.
Error 3021 (Submitting Insurance Claims)
If you receive error 3021 while submitting insurance claims in OfficeMate, the insurance type for the claim may be missing. To add the insurance type to the claim and clear the error, follow the instructions below and then refresh and resubmit the claim:
- In OfficeMate v8 or below, click Setup on the OfficeMate main window and select Business Names. In OfficeMate v9 or above, click Setup on the OfficeMate Administration main window and select Insurance Setup.
- Click the Insurance or Insurance Carrier tab.
- Select the secondary insurance carrier from the drop-down menu.
- Select the appropriate insurance type from the Type or Insurance Type drop-down menu.
- Click OK.
If you continue to receive error 3021 while submitting insurance claims, open the fee slip, ensure that the notes on the fee slip are correct and update the claim for the fee slip. In addition, ensure that the insurance carriers and plans on the claim do not contain any special characters and that box 17 has the correct values (i.e., DK, DN, DQ, etc.) selected before resubmitting the claim.
If there is no insurance carrier selected on the fee slip, add the insurance carrier to fee slip, record it, and resubmit the claim.
If, after completing all of the steps above, you continue to receive error 3021 while submitting insurance claims, return the items on the fee slip, create a new fee slip that contains the items, and submit a claim for the new fee slip.
Error -5001 : 0x80070002 (OfficeMate/ExamWRITER 8.0.25)
If you receive error -5001 : 0x80070002 while installing OfficeMate/ExamWRITER 8.0.25, re-download the installation file completely. You must ensure that the entire installation file is downloaded fully. It is 15.3 MB. If the file does not fully download and you attempt to install it, you may receive the -5001 error message.
User Account Control Error or Unable to Run .bat File to Fix Error 339
Follow the instructions below if, while using the OfficeMate Suite you receive the User Account Control error that states, "A program needs your permission to continue. If you started this program, continue." You will also need to complete the instructions below if you are receiving error 339 and you are unable to run ~OMRegAll.bat or ~Reg_9.0_OME_DLLs.bat, even as the administrator.
Windows Vista Users
- Click Continue
