If you upgraded to OfficeMate v9 and are using McKesson to file your insurance claims, you will receive a rejection on the Exclusion Report (FH0032C Missing/Invalid Service Facility ID) for your Medicaid claims unless you complete the steps below. In v9, unlike in previous versions, you must select a claim form preference to either print the location or Home Office address in box 32 on CMS 1500 forms; this preference, and the inability to leave 32b blank, is causing this error message and rejection. Our development team is aware of this issue and is working to resolve it in a future version of OfficeMate.
If you do not have service facility information (location number and facility ID):
- Open each Medicaid CMS 1500 form.
- Delete the address in box 32.
- Click OK.
If you do have service facility information (location number and facility ID):
- Open Home Office.
- Click Setup and select Location Maintenance.
- Double-click on the appropriate location.
- Select the correct qualifier (i.e., location number or LU) from the Qualifier (32b) drop-down menu in the Claims information box. Press F12 to add a qualifier to the drop-down menu options.
- Type the correct facility ID in the Facility ID text box. This ID must contain ten alpha/numeric characters, a dash, and then three alpha numeric characters (i.e., 1234567899-123).
- Click Save/Exit.
- Refresh and resubmit your claims.
